Laboratory Registration 實驗室註冊

  1. Lab Registration and Check Out Procedure
  1. 除了遵從校內正常註冊報到程序外，若使用ICMS實驗室必須通過ICMS實驗室註冊。
  2. 實驗室註冊遵循以下步驟, 並在LMIS系統完成。
  3. 實驗室固定每年八月底/九月初舉辦統一實驗室安全培訓並需通過筆試，合格者才可註冊成 正式使用者 FULL USER。如申請者在年度其他時間註冊，只能註冊成 臨時使用者 TEMPORARY USER，直至通過安全培訓及考核者才可轉為 正式使用者 FULL USER。
  4. 臨時使用者 TEMPORARY USER 身份不能申領實驗室資源和預約儀器，必須接受安全培訓及通過考核後轉為 正式使用者 FULL USER後才可開通。
  5. 曾經是 正式使用者 FULL USER 的用戶，畢業時已辦理註銷手續，但身份改變而重新進入本學院時，需要重新註冊LIMS新帳號，並重新簽署聲明書 (Declaration-Form)，並可豁免再次參加安全培訓。
  1. New comers not only should follow the regular Check-In/Registration procedures in the University, but also go through ICMS Laboratory Check-In/Registration procedure to become laboratory users.
  2. ICMS laboratory check-in/registration procedures can be completed in LMIS system.
  3. ICMS annual laboratory safety training which usually takes place in late August/early September every year. Only those who completed the ICMS annual laboratory safety training and passes the examination, are competent to be registered as FULL USER. If applicants register at any other time over the year, he/she may only be registered as TEMPORARY USER until ICMS annual laboratory safety training period.
  4. Applying & Collecting Lab Resources, Equipment Training & Booking are UNAVAILABLE for TEMPORARY USERS until they received and passed the Lab Safety Training Program to upgrade as FULL USERS.
  5. Fomer FULL USER who has completed the check-out procedures upon graduation or resignation, should register a new LIMS account when he/she enters ICMS again with a new identity. A Laboratory Safety Declaration Form should also be submitted. Safety Training can be exempted if already passed in the previous identity.

1.請下載及填妥下列表格，並登入本實驗室LMIS系統註冊新帳號及上傳表格：(申請者可使用PDF軟件合併所有文件至一個檔案才上傳至LMIS系統) 
Please fill in the following forms and sign up at LMIS system attaching the softcopy of the merged forms (.pdf) file to complete the registration procedure.

  • 如欲使用多個樓層的實驗室，需個別填寫該樓層的申請表並請相應樓層的技術員簽署後，掃描並合併至一個檔案後上傳到 LMIS系統繼續註冊流程。Registration Lab: Please fill in the respective laboratory floor (e.g. 5F/6F/7F/8F). To register into multiple floors, you will need to complete this form for each floor, and merge them into one .pdf file while uploading. Register a LIMS account.
  • 新註冊碩士生需附以下文件及完成２４０小時實習才可註冊成為實驗室臨時使用者（博士生以上不用交 These A, B attachments Only for MSc students）
    New Master Degree students should also attach the following documents with completion of 240 hours of laboratory probationary training

A.Application-Letter-for-Laboratory-Probationary-Training-Period-Laboratory
此表格作為課題組導師同意申請者到實驗室實習之憑證，申請者需由組內資深組員指導實習 This form provides an agreement from your supervisor, which agrees you will be led by an assigned senior student from your group.

B.Registration-Form-for-Laboratory-Probationary-Training
此表格作為申請者與課題組導師之間的實習時數記錄憑證，申請者需由組內資深組員指導實習 A record showing you have reached our requirement for probationary training.

2. 臨時使用者應參加實驗室安全培訓並通過考核合格後，轉為正式使用者以享有完整的實驗室資源。
Temporary Users should join the Laboratory Safety Training and pass the training examination to upgrade their account as Full User in order to enjoy comprehensive resources in the institute.

備註:

  • 非本校人員申請的審批將視乎該項目的安全性，以及ICMS實驗室的資源作考慮;  The approval of applications from non-university staff will depend on the safety of the project and the resources available in the ICMS laboratories.
  • 非本校使用者身份不能申領實驗室資源和預約儀器，也沒有門禁權限，但可經由甲方導師安排的正式使用者(Full user)協助申領或使用;  Non-University Users will not be able to apply for any laboratory resources, consumables and instrument reservation on their own. Party A (ICMS Principle Investigator) can arrange his/her Full User Staff to assist in resources implementation.
  • 申請表須經由本學院的導師代表向技術員申報。 The application forms MUST be submitted by Party A (ICMS Principle Investigator) to Laboratory Technicians for further registration procedure.
